Long before he formulated a cream, Des Fernandes was learning to operate on hearts. He trained as a cardiac surgeon under Christiaan Barnard, the South African who performed the world's first human heart transplant, before redirecting his scalpel toward reconstructive and plastic work in Cape Town. That clinical rigor shaped everything that came after: he approached the face the way a surgeon approaches tissue, asking what the cells actually need rather than what sells.
The turning point was grief. Early in his career he watched two young melanoma patients die before either reached the age of 23, and the helplessness sent him deep into the biology of how ultraviolet damage degrades skin. His reading kept circling back to one nutrient the skin steadily loses to light, pollution, and age, and by 1988 he had hand-mixed two creams to give his own patients. He insisted on doses high enough to genuinely work, an unfashionable stance when most cosmetic houses treated that nutrient as a label-friendly trace.
Demand from those patients outgrew his consulting room, and his sister, Val Carstens, took over the commercial side, selling first to friends and family and eventually steering the company to more than 140 products distributed across 70-plus countries. Fernandes stayed the scientific conscience, refining the graduated regimen that asks users to climb through rising strengths rather than shocking the skin, and engineering the fine-needled Roll-CIT device for at-home collagen induction.
The distribution model is deliberately old-fashioned: most ranges reach buyers through trained therapists who assess the skin first, which is why the catalog can feel opaque from the outside. That gatekeeping is the trade-off for a methodical, escalation-based system. Committed users who want structure get an unusually disciplined regimen; anyone hoping to grab a finished routine off a shelf will find the on-ramp slower and more involved than mass alternatives.
